Awards Eligibility and Criteria
Dr. Dana Winterburn Memorial Award
Dr. Dana Winterburn was a well-respected family physician in London and a devoted member of the Citywide Department of Family Medicine. Dr. Winterburn held special interests in palliative care, mental health care, and aboriginal health care. Over the course of her career, Dr. Winterburn provided comfort and care to many patients in all of these areas of care.
The Citywide Department of Family Medicine has established the Dr. Dana Winterburn Memorial Award in Dana’s memory. The scholarship will be awarded to a worthy family physician who meets specific criteria as outlined below. The scholarship will be presented at the Citywide Department’s Holiday Celebration held annually in December.
Criteria: The recipient must be a family physician member of the Citywide Department of Family Medicine. The recipient’s practice will demonstrate dedication to any or all of the following areas: mental health care, palliative care, and/or aboriginal health.
Process: A call for nominations will be distributed to members of the Citywide Department of Family Medicine yearly in advance of the annual Holiday Celebration. Nominations will be received from Department members and will include a brief description of the nominee and his/her practice with a focus on the criteria above.
The nominations will be reviewed by the Site Chiefs’ Committee of the Citywide Department of Family Medicine and a suitable recipient chosen.
In any one year, if no candidate meets the above-noted criteria, the Dr. Dana Winterburn Memorial Award will not be awarded.
Chair-Chiefs’ Recognition Awards
Two awards to be presented annually at the Citywide Department of Family Medicine’s Holiday Celebration held in December.
Award #1
Presented to a physician member of the Citywide Department of Family Medicine who, in the opinion of the Chair-Chief(s), has made a significant contribution to the Department in accordance with its mission statement and goals.
Nominations for this award may be submitted by any of the following:
- Member of the Citywide Department of Family Medicine
- Member of the Citywide Department of Family Medicine Executive Committee
- Staff member of any health care setting affiliated with the Citywide Department of Family Medicine (Family Medical Centres, hospital units, long-term care facilities)
Nominations should be forwarded in confidence to Sheena Blasing, Administrative Assistant, Citywide Department of Family Medicine at the following email address: sheena.blasing@sjhc.london.on.ca.
The deadline for receipt of nominations is November 15 of each year.
Award #2
Presented to an allied health professional or health team member who, in the opinion of the Chair-Chief(s), has made a significant contribution to the Citywide Department of Family Medicine in accordance with its mission statement and goals.
Nominations for this award may be submitted by any of the following:
- Member of the Citywide Department of Family Medicine
- Member of the Citywide Department of Family Medicine Executive Committee
- Staff member of any health care setting affiliated with the Citywide Department of Family Medicine (Family Medical Centres, hospital units, long-term care facilities)

Consultant of the Year
Criteria: Presented to a Consultant at London Health Sciences Centre or St. Joseph’s Health Care, London, who, in the opinion of the Chair-Chief(s), demonstrates exemplary patient-centered service and communication to referring family physicians.
Nominations for this award may be submitted by any of the following individuals:
- Member of the Citywide Department of Family Medicine
- Member of the Citywide Department of Family Medicine Executive Committee
- Staff member of any health care setting affiliated with the Citywide Department of Family Medicine (Family Medical Centres, hospital units, long-term care facilities)

The Award will consist of a plaque and a monetary donation made by the Citywide Department of Family Medicine to a recognized charity named by the recipient.
Dr. Keith Johnston Scholarship in Family Medicine
Criteria: In 1995 Victoria Hospital’s Department of Family Medicine established a postgraduate training scholarship for Department members wishing to pursue a program of professional development with London Health Sciences Centre (LHSC).
The Scholarship is named for the late Dr. Keith Johnston in recognition of his service to London Health Sciences Centre (previously Victoria Hospital) where he was a widely respected Chief of Family Medicine for many years.
The Dr. Keith Johnston Scholarship is awarded on an annual basis. The scholarship is intended to defray a worthy family physician’s expenses in completing a personal program of professional development for the duration of three or more working days. The program may include activities that are primarily education or research in nature.
Eligibility: Any family physician in Southwestern Ontario is eligible to apply for the scholarship.
Selected by: The Selection Committee
