Anonymous Reporting
The purpose of the OBGYN Transparent Resident Support Team's (TRST) Anonymous Reporting Form is to notify the team about concerns regarding yourself, a peer, colleague, or situation/incident within the OBGYN education programs.
You may be worried because you see a change in someone’s mood, behaviour, or coping strategies as a result of a stress, trauma, harassment or other life issues.
If your concern is about yourself, a response can only be given if your personal contact information is provided.
Please note this Anonymous Reporting Form is for members of the Department of OBGYN to advise TRST of non-emergency incidents of harassment, bullying and other safety issues.
