Windsor Trainee Accommodation Agreement

Trainee Accommodation Agreement
Between:
Western University , operating as Schulich School of Medicine & Dentistry – Windsor Campus, herein referred to as "Windsor Campus"
AND
Resident Physicians and Medical Students, herein referred to as "the trainee"
Whereas Windsor Campus has entered into a Tenancy Agreement with the Urban Enterprises Corporation, Medical Arts Building at 1011 Ouellette Ave, Windsor Ontario, which premises will be referred to herein as "Schulich - Windsor Campus Accommodations";
And whereas Windsor Campus has agreed to allow the trainee to occupy part of the Windsor Campus Accommodations during the period specified.
In consideration of being allowed to occupy the premises, and other good and valuable consideration, the parties agree as follows:
- During occupation of the premises by the trainee, the trainee will abide by the Schulich School of Medicine and Dentistry, The University of Western Ontario Code of Conduct and The University of Western Ontario Code of Student Conduct.
- The trainee will abide by the terms and conditions of the lease between the Windsor Campus and the Urban Enterprises Corporation.
- The trainee will be solely responsible for insuring his or her own personal property, and agrees that the Windsor Campus will not be responsible for any damage to that property during the trainee’s occupation of the premises.
- The trainee will be solely responsible for any loss or damage to the premises or the property of the Windsor Campus or the Urban Enterprises Corporation located on the premises, including any loss or damage caused by the trainee and his or her guests or invitees. The trainee agrees that any such loss or damage will be recovered by the Windsor Campus from the trainee in the same manner as outstanding fees or fines, including the sealing of the trainee’s academic record, until the loss or damage have been paid.
- The Windsor Property Manager will issue access keys to trainees at the commencement of training. Keys (including backup key) must be turned in or left with the Property Manager upon completion of training. The trainee is responsible for Windsor Campus Accommodation keys. Loss of these keys should be immediately reported to the Property Manager. Keys that are not returned and require courier back to Windsor will incur a $25 administration fee. The same fee will be incurred by the trainee for any keys lost, damaged or not returned upon completion of training in Windsor.
- The trainee will be responsible for ensuring all furnishings and property remains on the premises. The Windsor Campus will invoice the trainee for any costs of damaged or removed property. Failure to pay the invoice will be treated as a failure to pay a fee or fine pursuant to the UWO Academic Calendar. If extraordinary steps have to be taken to correct an accommodation issue, special notation will be made in the resident’s final evaluation with regards to lack of professionalism. Any and all issues will be escalated to the trainees Program Director.
- The Trainee also agrees to provide all towels and bedding for the entire duration of the rotation.
- Any issues, damage to the premises, furnishings or property (including any maintenance requirements with respect to plumbing, electrical, etc.) should be reported as soon as possible to the Clinical Education Team Leader.
- The trainee will not make long distance telephone calls billable to the premises while staying in the premises. Personal Cellular or third party credit card billing is suggested.
- No pets, reptiles, animals, fish, etc. are allowed in the premises at any time unless there is an approved need for a service animal.
- The trainee will be responsible and cognizant of utility costs and play a responsible role in controlling the Windsor Campus housing costs.
- The Urban Enterprises Corporation, Medical Arts Building is entirely a non-smoking building. Persons wishing to smoke must do so outside the building and away from buildings.
- There will be no alternation to the property (i.e. wallpaper, paint, etc.).
- The Windsor Campus reserves the right to make additional reasonable rules with respect to the use and occupation of the premises from time to time. Such rules, when communicated to the trainee in writing, shall be deemed to be part of this Agreement.
- The Windsor Campus reserves the right to inspect and have access to all units in the Medical Arts Building for maintenance within 24 hours of notice.
- The trainee will be expected to vacate the unit by 11:00a.m. on the last day of rotation. Any exceptions will require written notice and approval at least 2 weeks in advance.
- In addition to any other remedies available to it, the Windsor Campus may terminate this agreement for any breach thereof.
- Windsor campus reserves sole responsibility for adjudicating damage to the property rented to the trainee. The trainee is responsible for reviewing the state of the room and verifying that there have been no pre-existing damage and reporting any existing problems.
