Rebecca Bliss’ project seeks to understand and identify the mental health needs of the Hamilton Family Health Team patients and their intersection with social needs and social determinants of health. In partnership with the available data, the project proposes a qualitative study informed by constructivist grounded theory to further understand the depth of need directly from patients, providers, and those living within the different sub-regions of the HFHT. Once this data is coded and analyzed (via inductive reasoning) alongside environmental scans, gap analyses, and the available population health data, the results can provide the HFHT with the information it needs to contribute meaningfully to the structural resilience and mental health in the Hamilton Region among its patients, community, and partners.
